  • Support individuals with in-depth structured medication reviews
  • Ensure individuals have the correct medication and dosage

  • Work closely with Clinical Pharmacists to help individual manage common problems relating to their medication
  • Ensure patients are aware of how-to safety take their medication
  • Educate individuals on how to stay well with the right medication
