Eligible patients in the central Northampton and Wellingborough area’s will receive a invite from their GP practice, which will offer them 3 options of support. Patients can choose one or more from the options below.

If you are a patient in the central Northampton or Wellingborough area who interested in joining the programme, but has not yet received a text message, it may be that your GP practice has not yet signed up. Option 1) The MyHeart App

The myHeart app helps you look after your heart and stay healthy. You can use it to find helpful information, watch videos, check your medicines, and follow your care plan.

The app also helps you track your activity and symptoms, learn more about your heart condition, and understand how to take care of your health, all from your smartphone or tablet.

If you select this option, you will be given a log in and free access to the app.

Option 2) ISOBands and Exercise Booklet

ISOBands are special exercise bands that can help lower blood pressure. When you use them, your muscles tighten without moving. This can help your blood vessels work better and may lower your blood pressure.

The exercises are simple and don’t take much time. You only need to do four 2-minute exercises, three times a week, which adds up to just 24 minutes each week.

Studies have shown that regular use of ISOBands can help lower blood pressure. This may reduce the risk of heart problems, such as heart attacks and strokes. ISOBands are a simple, low-effort, and drug-free way to help keep your heart healthy.

If you select this option, you will be given a free ISOBand and exercise booklet. You will be invited to collect these from our trained health coach, who will demonstrate the bands to you.

Option 3) Group Health Coaching – Face to face or Online

This free 6-week course will help you learn how to manage your high blood pressure. Our trained health coach and nutritionist will guide you through the programme each week.

You will meet other people who also have high blood pressure. Together, you can share experiences and learn more about how healthy eating, regular activity, and managing stress can help keep your blood pressure under control.

Your coach will help you set small, realistic goals and build healthy habits that can make a real difference to your health.

The sessions are designed to fit around busy lives, with evening, weekend, and online options available.

If you select this option, you will be sent a booking link to select which a group session suits you. The sessions will be at the same day and time, for 6 weeks. We think you will get the best out of the sessions if you can attend as many as possible.
