What is Personalised Care?
Personalised Care means giving people real choice and control over their health and care, based on what matters to them. It is central to the NHS’s transformation, moving from a reactive, one-size-fits-all model to a proactive, patient-led approach.
The new NHS Long Term Plan commits to:
- Embedding the Comprehensive Model of Personalised Care, including shared decision-making, personalised care plans, social prescribing, supported self-management, and personal health budgets.
- Expanding roles such as health coaches, care coordinators, and link workers to provide time and support for what matters most to individuals.
- Leveraging digital tools and inclusive communication to ensure everyone can access and manage their care confidently.
- Building Neighbourhood Health Services that integrate health, social care, and community support to tackle health inequalities and improve outcomes.
The plan’s three big shifts – from hospital to community, analogue to digital, and sickness to prevention—are designed to deliver these outcomes by:
- Predicting and preventing ill health
- Integrated care pathways
- Empowering patients through choice and control

GPA have invested knowledge, skills, and resources, to enable our Personalised Care services to deliver quality care and support in our local population. Our staff have access to support, supervision, and training, which includes our monthly Communities of Practice and the Wellbeing Launchpad, our online directory of services.
